A US Coast Guard report has concluded that safety, testing, and maintenance failures at Oceangate were the primary causes of the 2023 Titan submersible disaster, which killed all five on board, including CEO Stockton Rush.

Former safety director David Lochridge, who was dismissed in 2018 after raising concerns, said the tragedy was foreseeable given the vessel’s deficiencies. He criticised both the company’s design and operations, as well as US authorities for not intervening sooner, claiming passengers “were sold a lie.”
